Официальное название
The Effectiveness of Itraconazole Pulse Therapy vs Topical 2% Ketoconazole in the Treatment of Moderate to Severe Seborrheic Dermatitis of Scalp. A Randomized Control Trial
Обзор
This study is a randomized controlled trial comparing the effectiveness of oral itraconazole pulse therapy versus topical 2% ketoconazole shampoo in treating moderate to severe scalp seborrheic dermatitis (SD). SD is a chronic inflammatory skin condition associated with Malassezia yeast, characterized by scaling, itching, and redness, significantly affecting quality of life. A total of 80 participants aged (14-70 years) with moderate to severe scalp SD will be enrolled and divided into two groups. Group A will receive oral itraconazole (pulse regimen over 6 months), while Group B will use ketoconazole shampoo (initial daily use followed by twice weekly application). Outcomes will be assessed using the Scalp Seborrheic Dermatitis (SSD) score, Clinical Global Impression (CGI) scale, and Patient Global Impression (PGI) scale. The study aims to compare: Treatment effectiveness Frequency of disease relapses Patient satisfaction Side-effect profiles Data will be analyzed using statistical tests (t-tests and chi-square), with significance set at p \< 0.05. The hypothesis is that oral itraconazole pulse therapy is more effective than topical ketoconazole in controlling symptoms and preventing relapses in scalp SD.
Подробное описание
Seborrheic dermatitis (SD) is a chronic, relapsing inflammatory skin disorder that primarily affects sebaceous gland-rich areas such as the scalp, face, chest, and upper back. It presents with greasy scales, erythema, itching, and dandruff, which can significantly impair quality of life. Globally, SD affects approximately 4.38% of the population, with higher prevalence in adults and males.
Although the exact pathogenesis remains unclear, strong evidence highlights the role of Malassezia species in disease development. These yeasts metabolize skin lipids, producing irritant free fatty acids (such as oleic acid), which trigger inflammatory pathways involving cytokines (IL-1β, IL-4, IL-8, IL-17) and NF-kB activation. Microbial imbalance, genetic predisposition (HLA associations), and links with systemic conditions (e.g., HIV, Parkinson's disease, psychiatric disorders) also contribute.
Rationale of the Study Current management focuses on symptom control and prevention of relapses. Antifungal therapies remain the cornerstone of treatment due to the central role of Malassezia.
Topical ketoconazole is widely used and effective but may cause local irritation in some patients.
Oral itraconazole offers both antifungal and anti-inflammatory effects and may provide better long-term control, especially in moderate to severe cases.
However, there is limited comparative evidence between these two approaches in scalp SD. Therefore, this study aims to identify a more effective and practical treatment strategy.
Aim and Objectives The primary aim is to compare oral itraconazole pulse therapy with topical 2% ketoconazole shampoo in managing moderate to severe scalp SD.
Specific objectives include:
Comparing treatment effectiveness Evaluating reduction in disease relapses Assessing patient satisfaction Comparing side-effect profiles Hypothesis Oral itraconazole pulse therapy is more effective than topical ketoconazole shampoo in treating and preventing relapses in moderate to severe scalp SD.
Operational Definitions Severity Measurement: Using the Scalp Seborrheic Dermatitis (SSD) score (0-16), which assesses flaking, erythema, and itching.
Moderate to Severe SD: SSD score \>5
Effectiveness: Determined through:
SSD score improvement Clinical Global Impression (CGI) scale (clinician-rated) Patient Global Impression (PGI) scale (patient-reported) Methodology Study Design Randomized Controlled Trial (RCT) Study Setting \& Duration Dermatology OPD, Combined Military Hospital Bahawalpur Duration: 6 months after approval Sample Size Total: 80 patients (40 per group) Calculated using statistical software with 95% confidence level and 80% power, adjusted for dropouts Sampling Technique Non-probability randomized sampling Participants allocated via lottery method Eligibility Criteria
Inclusion Criteria:
Age 14-70 years Moderate to severe scalp SD Willing to participate
Exclusion Criteria:
Cardiac disease Liver disease Pregnancy or lactation Uncontrolled hypertension Polypharmacy (drug interaction risk) Drug hypersensitivity Intervention Groups
Group A (Itraconazole):
100 mg twice daily for 1 week (first month) Then 100 mg twice daily for first 2 days of each month (next 5 months)
Group B (Ketoconazole):
2% shampoo daily for 14 days Then twice weekly for 5 months Data Collection Procedure Baseline data: demographics, disease duration, flare frequency, photographs
Disease severity assessed using SSD score at:
Baseline 3 months 6 months
Final evaluation includes:
CGI score (by blinded dermatologist) PGI score (by patient) Side effects (e.g., nausea, dermatitis, jaundice) recorded throughout Outcome Measures Primary: Improvement in SSD score
Secondary:
CGI and PGI scores Frequency of relapses Side effects Patient satisfaction Data Analysis Plan Software: IBM SPSS 29.0 Descriptive statistics: Mean, standard deviation, etc.
Inferential tests:
Independent t-test (between groups) Paired t-test (within groups) Chi-square test (categorical variables) Statistical significance: p \< 0.05 Expected Outcome The study is expected to determine whether oral itraconazole provides superior control of symptoms and relapse prevention compared to topical ketoconazole, potentially offering a more effective treatment strategy for moderate to severe scalp SD.
